Schedule an Assessment

After the initial call with our admissions supervisor, we’ll schedule your client for a full clinical assessment with one of our licensed clinicians.

This typically occurs on-site at Sandstone Care and takes 60–90 minutes. The assessment clinician will conduct a full biopsychosocial assessment to collect the following information:

  • Presenting concerns
  • Psychiatric history
  • Medical history
  • Past treatment history
  • Information about referring treatment providers

Please note: any out-of-state clients will complete a pre-assessment over the phone, which usually takes 30–60 minutes to confirm the appropriateness for our treatment programs.

Stay in Touch

Following a client’s admission into Sandstone Care, we will contact the referring treatment professional within 72 hours. We’ll ask the client to sign a release of information for his or her outside treatment professionals upon intake.

If the client signs the release, our primary therapist will reach out to further discuss the client’s case and provide updates as requested by the referring professional.

Please note that if you do not receive communication from Sandstone Care, it is likely due to the client not signing a release of information. We respect and honor our client’s right to confidentiality; however, we view this as a clinical issue that we will work to address during treatment.
