What Are Examples of Secondary Trauma?

A common example of secondary trauma is when individuals in helping professions, such as nurses or first responders, experience symptoms by witnessing or hearing stories of other people’s trauma over and over again.

For example, a social worker who consistently is tasked with checking in with foster children about the safety of their home environments may experience secondary trauma after years of witnessing the effects of neglect.

It is important to note that they could develop secondary trauma even if they experienced neglect in their own life.

They may feel anxious, helpless, and fall into despair when they cannot help in tragic situations over and over again.

This is especially true of those who work in environments where they are not provided with the proper mental health care and training to be able to process the situations they encounter.

 

What Is Secondary Trauma in Social Work?

Social workers might experience secondary trauma, also sometimes referred to as compassion fatigue, from feeling the pain and stress of the people they are helping.

In Compassion fatigue: Coping with secondary traumatic stress in those who treat the traumatized, Dr. Charles Figley, C.R defined secondary trauma as “the natural consequent behaviors resulting from knowledge about a traumatizing event experienced by a significant another. It is the stress resulting from helping or wanting to help a traumatized or suffering person”.

The Secondary Traumatic Stress Scale (STSS) is used as a self-report for professional caregivers to assess the frequency of secondary traumatic stress symptoms.

There are also different exams and tests that may be done by a healthcare professional if you think you might have secondary PTSD.

Someone with secondary trauma can experience symptoms of PTSD. However, the two are not the same.

The difference is that PTSD occurs from direct experience with trauma, whereas secondary trauma comes from indirect exposure to trauma.

A person with secondary trauma can experience symptoms that are similar to those of PTSD symptoms, however, the conditions are very different.

Burnout occurs when a person experiences exhaustion and a lack of interest and motivation regarding their work.

However, burnout can also occur when someone has a lot going on in their life, whether it’s work, school, or personal life.

Burnout differs from compassion fatigue because it refers to general exhaustion from work. However, compassion fatigue commonly causes burnout, especially when there are not mental health care interventions available for employees in high-risk professions.

Sometimes, secondary trauma is also referred to as compassion fatigue.

Compassion fatigue is a term used to describe the symptoms that occur when a person whose profession involves prolonged exposure to other people’s trauma.

Symptoms of compassion fatigue can include exhaustion, sleep disruptions, anxiety, headaches or body aches, numbness, emotional disconnection, and difficulty with personal relationships.

One way to mediate, prevent, or improve the symptoms of compassion fatigue is through compassion satisfaction. Compassion satisfaction is the pleasure that a person in a helping profession gets when they are able to help or contribute to other people’s well-being.

Burnout can share many similar symptoms and causes with secondary traumatic stress.

Burnout can occur when a person is undergoing prolonged emotional, physical, and/or mental stress. It is often related to work, but it can also relate to school or other parts of someone’s daily life.

The major difference between the two is that burnout is not solely caused by prolonged exposure to other people’s trauma.

The secondary traumatic stress scale involves three categories: avoidance, intrusion, and arousal.

Individuals can assess themselves, take scores, and add them up. Higher scores indicate a higher frequency of symptoms.

Secondary exposure to trauma is considered a stressor for post-traumatic stress disorder in the Diagnostic and Statistical Manual of Mental Health Disorders, Fifth Edition.

The ABCs are used to help manage symptoms of secondary trauma.

“A” stands for awareness.

Acknowledging your history of trauma exposure and also taking inventory of balance in your life is important in being aware of when your mental health is being impacted.

A big part of awareness is taking care of yourself and making sure you prioritize self-care. It’s also important to be debriefing your bosses or loved ones about what you are struggling with so that they can fully support you when you are in need.

“B” stands for balance.

While it is healthy not to be overly involved with individuals you work with when you are in a helping profession, you also still have emotions because you are human. Allow yourself to process these emotions and not push them to the side or “bottle them up.”

It is also important to maintain clear boundaries. Maintaining a healthy work-life balance can help someone enjoy time by themselves to destress or to hang out with people they love.

If you find it difficult to destress or take time away from work or from other people, it can be helpful to find a new hobby or leisure activity to try.

Lastly, “C” stands for connection.

Sometimes, the people that help others don’t ask for help for themselves.

Staying connected with a support system, such as your loved ones, friends, or community, is important in making sure you don’t take on all this stress on your own and that you get help if you need it.
