What is Transitional Living?

Transitional Living for Young Adults

Maybe you feel out of place, surrounded by triggers, or you just aren’t ready to face the stresses of life at home. If you’re feeling this way, you are not alone.

At Sandstone Care, we understand what you are going through, so we created transitional programs for young adults that give you the supported independence you need to rebuild confidence, re-establish your identity, and set goals for the future.

You will participate in 25 hours a week of day treatment, also known as a partial hospitalization program (PHP), while being immersed in a structured, therapeutic living environment for 2 – 6 months.

Who is a Transitional Living Program (TLP) For?

If you are a young adult man or woman between the ages of 18-30 and are in need of guidance and support as you transition into independent living, then this program is for you.

Our team of experienced professionals includes licensed therapists, addiction specialists, and mental health professionals who are dedicated to helping you achieve your recovery goals.

Eligibility for this friendly group home setting depends upon a few factors, including if your living situation puts you at risk for relapse, you’d benefit from being surrounded by supportive community resources, or you need extra support strengthening basic life skills.

We provide supportive services and life skills training to help you master money management, achieve job readiness, and have coping strategies for the real world.

This independent living program will benefit you if you:

Are stepping down from a medical detox program
Struggle to live independently
Need support in developing healthy lifestyle habits

Living Skills from Our Transitional Program

Transitional living gives you a safe, controlled setting to learn the skills you need to reach your goals before you start living independently. Some, but not all, of those skills include:

  • Emotional resilience: Learn to manage stress, reframe thought patterns, and face emotional challenges without turning to addictive behaviors.
  • Healthy communication: Heal your relationships by learning how to take accountability, set boundaries, and ask for help.
  • Confronting triggers: Identify and overcome triggering situations and environments to stay sober longer.
  • Self-sufficiency: Cultivate independence and self-confidence so that you can withstand peer pressure.
  • Employment & academic skills: Find job opportunities, budget, and study efficiently to build a strong foundation for life beyond addiction.

Day Treatment (PHP)

Our PHP (Partial Hospitalization Program) complements the transitional living setting by giving you the therapeutic support you need to recover.

While transitional living gives you life skill learning and support groups, day treatment will give you highly structured support with evidence-based methods like cognitive behavioral therapy.

What is IOP?

Intensive Outpatient Program (IOP)

The 8-12 week IOP, available on-site or virtually, is vital for you while you are in our transitional program for young adults.

Unlike the day treatment option, IOP is tailored for post-school or post-work hours, our intensive outpatient program supports young adults in managing mental health and substance use challenges while juggling daily responsibilities.

We offer personalized evidence-based treatment, emphasize family involvement, and provide the tools necessary for achieving lasting wellness.

Our Medically-Assisted Detox and Inpatient Center offers private rooms and 24/7 medically supervised care to support a safe recovery from drugs and alcohol, followed by comprehensive treatment that addresses your physical, psychological, emotional, and spiritual needs.

2-6 Months

Our Transitional Living Program combines the structure of treatment and group living, with the freedom of living away from home.

This level of care is for young adults (18-30) who are not quite ready to heal from home. This level of care provides a safe and supportive living environment integrated with a PHP or IOP level of care. Here, you’ll learn life skills and healthy recovery strategies in an environment that’s set up for your success.

4 weeks of on-site day treatment.

Our Young Adult Day Treatment Program, also known as Partial Hospitalization Program (PHP), is a highly structured level of care for young adults that offers five days of robust programming a week.

Our two distinct mental health and substance use tracks help young adults to stabilize, begin to understand their mental health and/or addiction struggles, and heal from them. We strive to help our clients become more like the person they want to be without using negative coping strategies or substances to get there.

12 weeks of on-site or virtual treatment.

Our Young Adult IOP, or Intensive Outpatient Program, offers two distinct tracks to address young adult needs, each track consisting of 3-4 days of weekly programming.

In our mood disorder track, we are able to focus on mental health, depression, trauma, and anxiety. In our dual diagnosis track, we are able to support young adults with substance use and mental health challenges. Each focuses on developing positive social, academic, and vocational habits while continuing with their job or school responsibilities.
